Passports can be renewed if ALL 4 of these conditions are met:

Most recent passport was issued when you were age 16 or over, AND

Most recent passport is STILL in your possession, AND

Most recent passport is undamaged, AND

Most recent passport was issued LESS than 15 years ago.

Passports must be renewed if running out of blank visa pages.

Passports must also be renewed if they are MORE THAN ONE YEAR OLD AND STILL VALID and the applicant  needs to change the name in their passport, OR make any sort of amendment in a passport (incorrect /change information), OR extend a 1 year temporary passport that is extendable.

You must provide your most recent, undamaged passport. 

This Letter of Authorization gives Visa Obtainers permission to represent you at the Department of State. We cannot complete your order without the letter of Authorization.

Each Letter of Authorization MUST be completed by handwritten.

Original signature is required.

All fields are required to be filled in the Letter of Authorization.

Check the first two boxes , DO NOT check the third box , this will cause delay.

In the courier company name field, please handwrite Visa Obtainers Inc.

Do not cross out, white out or print over as this will case your passport order to be suspended. please use black ink when signing the Letter of Authorization

One passport type photograph is required. Photos must be recent, taken within the last six months, 2x2 inches, front view, full face, in color and on plain/white background.

Copy of travel itinerary from airline or travel agency with applicant name, travel date and flight information OR

Copy of Airline ticket showing foreign departure, OR

If traveling for business and neither of the above is available, a business letter can be used

Please note that proof of departure is not required for regular service.

If  you require a name change

You must submit the official document(s) indicating name change

These include marriage certificate, divorce decree (specifically stating name change), or court paper

The Passport Agency does not accept photocopies of any of these forms, official documents will be return with your new passport

The online application has replaced  Hand Written applications. No hand written forms will be accepted.

You must complete this form electronically  (failure to use this online application form may delay the issue of your passport)

The last page of the application is a print summery page, please scroll to the bottom left corner of the page and click CREATE FORM you will then be able to print.

Do not forget to sign and date the application once it has printed.

Print single side only on two sheets of white paper.

If you applying for a passport card, please mark the box next to "passport card" on your passport renewal application

There is additional $ 30 government fee for the passport card. 

The passport card cannot be expedited at this time and will be mailed to you directly by first class mail from the US Passport Agency

Government fees

A check or money order in the amount of $ 209.53 for passport book only and $ 239.53 for passport book and card to US Department of State.

If using a personal check a pre print name and address MUST be located in the upper left hand corner.

Write your name and date of birth in the memo section. 

If you are paying by credit card, please complete the credit card information in the online order form.
